Resumen
Interoperability challenges in healthcare often result in fragmented patient records and duplicated procedures, affecting continuity of care and operational efficiency. While numerous theoretical frameworks and standards exist, practical implementations remain limited, with few complete applications enabling secure and controlled data exchange. Motivated by this gap, we developed Wirachain, a decentralized application that integrates blockchain technology with the HL7 FHIR standard to provide a functional environment for electronic health record management. The system supports patient-controlled permission granting and revocation, allowing healthcare providers to access clinical data only when authorized. It incorporates authentication, role-based access, and persistent storage in both FHIR-compliant and traditional databases, enabling practical testing of clinical workflows. Validation was performed using synthetically generated data and stress tests to evaluate system performance under load. Results demonstrate that the application can reliably manage permissions and data access, supporting interoperable clinical operations while maintaining efficiency and security. This work illustrates that fully implemented, blockchain-based solutions can bridge the gap between conceptual frameworks and practical healthcare interoperability.
