Resumen
Bronchobiliary fistulas (BBF) are abnormal communications between the biliary tract and the bronchial tree, considered potentially serious. This report describes a transdiaphragmatic surgical approach to a BBF secondary to a hepatic hydatid cyst, a technique rarely documented in the literature. We report a case of a 52-year-old male from an endemic area presented with dyspnea, yellow expectoration (bilioptysis), and hypoxemia. BBF was identified by computed tomography and sputum analysis. A right thoracotomy and diaphragmatic opening were performed to evacuate the hepatic cyst and close the fistula. The patient had a favorable recovery and was discharged without complications. The transdiaphragmatic approach proved to be an effective surgical option in this case of BBF secondary to hydatidosis. Timely detection and multidisciplinary management were key to successful treatment. © (2025), All rights reserved.
