Resumen
Introduction. Prostate cancer is the second most frequently diagnosed male neoplasm worldwide. Radical prostatectomy is the surgical treatment of choice; currently with robotic assistance. Objective. To describe the initial experience and analyze the results of radical prostatectomy assisted by the Da Vinci robotic system at the Instituto Nacional de Enfermedades Neoplásicas. Methods. A descriptive study was conducted of the first 40 cases performed by two surgeons between December 2024 and March 2025. The clinical and pathological histories of the patients undergoing prostatectomy were analyzed, as well as the results and perioperative times. Results. All cases of radical prostatectomy were completed successfully, without the need for conversion, with minimal operative bleeding. The average age was 65 years, the average preoperative prostate antigen was 15.8 ng/mL, the cribriform pattern of the biopsy was present in 20% of patients, and the Gleason score of 4+3 was present in 40% of cases. In the surgical sample, perineural invasion was present in 90% of cases, lymphovascular invasion in 32.5%, the average number of lymph nodes resected was 13.1, and 52.5% were TNM stage III. The average intraoperative blood loss was 303.5 mL. The times in minutes for surgeon positioning, trocar placement, coupling, and console time were 19.1, 10.5, 8.8, and 209, respectively. Conclusions. During the study period, robot-assisted radical prostatectomy performed optimally in terms of results and operating times. © (2025), All Rights Reserved.
