Resumen

Introduction: Apgar score is one of the main indicators for performing neonatal resuscitation. Objective: To determine the perinatal factors that predict a low Apgar score at five minutes in neonates attended in an EsSalud I-4 facility. Methods: Observational, analytical case-control study. A census analysis was carried out of all the newborns attended at the Octavio Mongrut Munõz hospital from the 2009-2018 period, after verifying the inclusion and exclusion criteria. The EsSalud Perinatal Surveillance System (SVP) was used. The Chi square test and crude odds ratio (ORc) were used as a measure of association. Likewise, binary logistic regression was applied to estimate adjusted odds ratios (ORa). Results: In the bivariate analysis, the greatest risk factor was neonatal weight <1500 grams (ORc = 18.77; p <0.01), then the presence of congenital anomaly (ORc = 5.81; p <0.01), aspiration of meconial amniotic fluid was found. (ORc = 4.69; p <0.01), neonatal weight between 1500-2499 grams (ORc = 2.51; p <0.01) and the presence of intrauterine infection (ORc = 2.34; p = 0.01). However, in the multivariate analysis and after adjusting for gestational age, only aspiration of meconial amniotic fluid (ORa = 7.82; p <0.01) and the presence of congenital anomaly (ORa = 3.19; p <0.01) were factors predictors of a low neonatal Apgar score at five minutes. Conclusion: Aspiration of meconial amniotic fluid and the presence of congenital anomaly are predictive perinatal factors of low Apgar score at the fifth minute of extrauterine life in neonates of an I-4 EsSalud hospital.
