Resumen
Adrenal cortical carcinoma is an extremely rare paediatric malignancy which usually presents with hormonal manifestations. The diagnosis is generally advanced, and the disease course is aggressive. Imaging plays a crucial role in diagnosis, preoperative planning, and staging. We present the case of a 7-year-old patient with a tumour that compromised the entire left renal parenchyma, with no evidence of adrenal tissue on computed tomography; the initial clinical diagnosis was Wilms tumour. However, the histopathological study reveals an adrenal cortical carcinoma with p53 overexpression; Unfortunately, the prognosis was ominous, and the patient passed away 15 months after surgery. © (2025), (Anales de la Facultad de Medicina).
