Resumen
Emphysematous gastritis is a rare condition with a high mortality rate, particularly in older adults. Currently, the literature is limited; therefore, case reports are valuable for describing diagnostic approaches and early interventions. The presence of gas in the intrahepatic portal venous system is an uncommon radiological finding. Up to 50% of cases may be associated with intestinal pneumatosis (the presence of gas within the intestinal wall), presumably as part of the evolution of the same pathophysiological process. The aim of this report is to discuss, based on imaging findings, the coexistence of emphysematous gastritis, portal venous pneumatosis, and intestinal air, as well as their possible etiology and differential diagnosis. © (2025), All Rights Reserved.
