Resumen
Objetive: identify the production of ESBL (extended spectrum beta-lactamases) and carbapenemase genes blaKPC, blaNDM and blaVIM in E. coli and K. pneumoniae from the “Regional Institute of Neoplastic Illnesses North, Trujillo – Peru”. Methods: 20 strains of K. pneumoniae and 69 of E. coli and were studied, in which resistance to carbapenems, production of ESBL and carbapenemases were determined; Then, by means of the double disk synergy technique, using phenyl boronic acid (APB) and ethylene diamine tetraacetic acid (EDTA), class A and B of carbapenemases were determined, respectively. The positive cultures were investigated for the presence of the blaKPC, blaVIM and blaNDM genes. Results: Regarding E. coli, it was determined that 72.4% were resistant to carbapenems; Of them, 82% had ESBL and 28% had carbapenemases. In K. pneumoniae, 75% were resistant to carbapenems; of them, 86.7% expressed ESBL and 26.7% carbapenemases. Of those who expressed carbapenemases in E. coli, 35.7% were from group A, 57.1% from group B and 50% revealed blaVIM. In K. pneumoniae, 75% were from group A, 25% from group B and 25% expressed blaVIM. An E. coli culture coproduced ESBL, blaVIM and blaNDM. Conclusions: A high frequency of ESBL and carbapenemases was revealed in both bacteria. It is proven that the same crop can coproduce more than one resistance mechanism to beta-lactams.
