Resumen

This paper proposes a Digital Transformation Roadmap applicable to organizations across different sectors, integrating Lean-Agile practices, digital maturity models, and change management strategies. The roadmap is structured into five progressive phases —recognize, rediscover, redefine, renew, and reimagine— and addresses key dimensions such as processes, people, data, and technology. Through OKR-based metrics and innovation portfolio management, this model provides a practical guide to implementing sustainable, results-oriented digital initiatives.
