Correlation between the estimating equations of glomerular filtration rate and endogenous creatinine clearance: primary care

Torres-Ramos L.E.; Apaza-Tapia R.M. · Revista de Salud Publica (2024) · DOI: 10.15446/rsap.v26n2.102498

Resumen

Objective To determine the correlation between the equations for estimating glomerular filtration rate MDRD (Modification of Diet in Renal Disease) and CKD-EPI (Chronic Kidney Disease Epidemiology Collaboration; and endogenous creatinine clearance (ECD) with 24-hour urine collection in patients from a primary care center in Tacna, Peru. Methods Non-experimental, cross-sectional and retrospective research; 462 patients who met the inclusion criteria between 2017 and 2019 were included; Spearman's Rho statistic was applied for the correlational analysis. Results The MDRD and CKD-EPI estimating equations describe a moderate correlation with mGFR; between both equations there is a very high correlation. The correlation is directly proportional to age; moderate correlations are found in the 60 to 74 age group: MDRD-4 (rho = 0.52) and CKD-EPI (rho = 0.53). In the group with mGFR ≥ 60 ml / min / 1.73 m², the correlations are low, while in the group of patients with mGFR <60 thousand / min / 1.73m², the correlations are moderate. In patients with chronic kidney disease (CKD) the correlations are high: MDRD (rho = 0.74) and CKD-EPI (rho = 0.76). Conclusions The correlations found between both equations and the DCE are acceptable, but not optimal; CKD-EPI did not show a better correlation than MDRD. Despite the high correlation between them, the use of MDRD is suggested in the absence of a standardized creatinine technique and the use of ECD with caution, improving the strategy of obtaining 24-hour urine, in the prevention of CKD within the scope of public health.
